Masshealth GAFC
Under the MassHealth Group Adult Foster Care (GAFC) program residents receive personalized support to help manage evolving needs. Staff assist with cueing and supervision through tasks, from approx. 4:30pm-9:30pm, 7 days a week, including:
- Grooming: Ensuring regular grooming routines are maintained.
- Dressing: Assisting with dressing appropriately for the weather and season.
- Personal Hygiene: Providing support with safe bathing, ensuring.thoroughness, appropriate time amount and regularity.
- Eating: Helping maintain a safe nutritional diet, considering factors such as weight management, food allergies, energy levels and dental health.
- Prepared dinners are available to residents each evening.
- Bathroom Use: Promoting healthy bathroom habits, including thorough cleaning, hand washing, and leaving the space sanitary for others.
- Nursing Oversight: Professional nursing guidance and care.
- Bi-Monthly R.N. Home Visits: Regular check-ins from a registered nurse.
- Light Case Management: Connecting residents to community resources.
Other Support
- Live-In Staff: 365, 10pm-8am, evening/morning check ins and overnight emergencies.
- Weekly House Meetings: Staff-led meetings to encourage communication and community.
- Private Supplemental Staff: Residents are welcome to complement the staffing model at Cabot House with other types of unrelated staffing such as: DDS: Individual Living, Adult Companion, Self-Directed, Remote/Telehealth Support. Additionally, Private Life Coaching, Personal Trainers, Social/Recreation Partners, VNA Medication Management, etc.)

- Daily Staff-Prepared Dinner: Enjoy a freshly made meal each evening.
